Febreze
I'm not sure about a couch, but when I bought a smelly used car, I bought a few bottles of the odor neutralizer spray and used it every time I got in for a few days (and aired the thing out). I would assume the interior of a car is sorta the same as a couch. Best I can think of--good luck!

It doesn't say specifically that it removes cigarette odor, but it's never failed me yet. It's suitable for upholstry, but I'd do a fabric test in a hidden spot first to make sure the fabric dye doesn't react to the enzymes.

I also think odor
I also think odor neutralizer spray should do the trick.
Vamoose
I used Vamoose on some used furniture I bought and it works great.
